Thanks for the report! Sorry you ran into this problem. This is a rare bug in 3.8.99 about when attachment vertices are updated. Do you remember exactly what you were doing when it happened? For example, where you were clicking, if you were dragging, etc?
It may be fixed in v4, which is currently in beta.
